My acknowledgements are many and varied and I'd like to apologise for
omissions in advance. Firstly I'd like to thank my darling wife, Monica, for her
patience with me during the writing of this book. Following quickly to avoid
trouble I must also thank my Mum for her priceless contributions, dodgy
spellings and all. Then there's my Dad and my vast unbelieving family for,
